Abstract:

AMNIOTIC FLUID VOLUME (AFV) is one of the important parameters in the assessment of fetal well-being. The ability of ultrasound measurements to represent the actual AFV is unproven. This study was undertaken to compare correlation of conventional AMNIOTIC FLUID index (AFI) and radial AMNIOTIC FLUID index (RAFI) as a new method with actual FLUID VOLUME on phantom. As an experimental study, 10 to 100 ml of water with 5 ml intervals was injected to a rubber bladder as a uterus phantom containing a 15 week gestational age fetus. The vertical diameter was measured in largest FLUID pouch at each quadrant. Four diameters were summed as conventional AFI. The largest radial diameter perpendicular to uterus and fetus was measured at four quadrants and were summed as RAFI. Databases were analyzed based on correlation and regression methods. RAFI and conventional AFI predicted 91.6% and 65% of variations of FLUID VOLUME, respectively (P < 0.001). In conclusion, RAFI is more accurate and reliable than conventional AFI in the prediction of injected FLUID VOLUME.

BACKGROUND AND OBJECTIVE: Non Stress Test (NST) is the most common test before delivery that is done in some cases with probability of fetal distress. The objective of this study was to determine the sensitivity and specificity of NST in cases with complaint of decreased fetal movement or decrease of AMNIOTIC FLUID VOLUME according to sonography report.METHODS: This cross- sectional study was done on 120 women referred to Bahonar hospital in Kerman. It was performed on all pregnant women with gestational age of 38-42 who complained of decrease of fetal movement and a decrease of AMNIOTIC FLUID VOLUME was reported in their sonography. FINDINGS: The result of NST in 64.1% of patients was nonreactive and in 35.9% was reactive. 41.7% of cases were with fetal distress during labor or delivery, of which 94.1% had nonreactive NST. The sensitivity and specificity of NST in cases with decrease of AMNIOTIC FLUID were 89% and 74.5%, respectively and in cases with complaint of decreased fetal movement were 63.4% and 32%, respectively.CONCLUSION: Exact heart rate monitor in cases with complaint of decreased fetal movement or AMNIOTIC FLUID VOLUME and also with nonreactive NST is necessary

Abstract:

aIntroduction: Metritis is one of the causes of maternal morbidity and there are many risk factors for it. Because of the importance of possible association of meconium-stained AMNIOTIC FLUID and maternal metritis the present study was done to determine whether meconium stained AMNIOTIC FLUID (MSAF) is associated with puerperal infection. Methods: In this case-control study 150 women with MSAF and 150 women with clear AMNIOTIC FLUID (gestational age > 37 weeks) were compared for puerperal metritis. Results: Subjects’ educational level, gestational age, gravida, mean vaginal exams, route of delivery and mean neonatal weight were not significantly different between the two groups. But mean maternal age and neonatal Apgar were significantly different in the two groups. That is, post partum metritis in women with clear AMNIOTIC FLUID occured less than women with MSAF (2.7% vs 10%, P<0.014).Conclusion: Since meconium stained AMNIOTIC FLUID is associated with metritis, special care in this regard is recommended.

Abstract:

Non Stress test is the most common well being test before delivery that is done in probable cases of fetal distress. Decision for continuation or terminating the pregnancy is based on the result of NST along with some other criteria. The aim of this study was to determine the sensitivity and specificity of NST in cases with decreased fetal movement or decreased AMNIOTIC FLUID VOLUME on the basis of sonography, report. This cross-sectional study was carried out on a group of 120 women who had referred to the Niknafs clinic of Bahonar hospital in Kerman. Mean age of patients was 25.7±4.8 years old, mean gravidity and pariety was 2.3±1.3 and mean gestational age was 40.3±1.1 weeks (38-42 week). 1.1 (84.2%) of mothers were complaining of decreased fetal movement and 61 % of them showed decreased AMNIOTIC FLUID in their sonographies. The result of NST in 64.1% of patients was nonreactive and in 35.9% was reactive. In 41.7% of cases of fetal distress was during labor or delivery of which 94.1 % had nonreactive NST. The sensitivity and specificity of NST were respectively 89% and 74.5% in cases that decreased A.F had been reported in sonographies, and in cases with compliment decreased fetal movement by mothers. The sensitivity and specifity of NST were 68% and 32% respectively.

Abstract:

Background and Objectives: Oligohydramnios is a deficiency in the amount of AMNIOTIC FLUID, lower than expected for the pregnancy age concerned. In severe cases this may lead to fetal morbidity and mortality. The purpose of this study was to determine the effect of oral and intravenous hypotonic liquid on the AMNIOTIC FLUID index in pregnant women.Materials and Methods: This clinical trial study was done in 60 pregnant women in their 35th to 38th weeks of pregnancy with AMNIOTIC FLUID indices of 5 to 10 cm. The AMNIOTIC sacs in all these women were intact and sonograms showed no abnomalies in their fetuses. They were divided into 3 groups: The first group did not receive any treatment and was designated as the control group. The second group was treated by oral hydration (2lit/2h) and the third group was treated by intravenous (Ringer) Isotonic liquid (2lit/2h). A sonography was administered and the AMNIOTIC FLUID index was calculated prior to and 1 hour after treatment. Results: In the control group, the difference of the mean of AMNIOTIC FLUID index prior to and after treatment was 0.39±1.74 cm (P-Value=0.33). In the second group the difference of index before and after treatment was -1.32±1.82 cm (P-Value=0/004). In the third group the difference of the mean of the AMNIOTIC FLUID index before treatment and after treatment was -1.85±1.85cm (P-Value=0.001). The differences in the AMNIOTIC FLUID index was significant (p=0.001).Conclusion: The results of our study showed that the administration of oral and intravenous maternal hydration is a suitable procedure for the short-term increase of the AMNIOTIC FLUID index. This increase was higher in treated group.
